Complex cases

Full-mouth and complex reconstruction — metal-free

For demanding cases — full-arch rehabilitation, multiple implants, bone loss, or failed and corroded previous work — rebuilt in Madrid entirely without metal, by surgeons. International patients travel to Bio Salud Dental for reconstruction done to the highest standard, with a clear plan agreed before they arrive.

For the cases other clinics call “too complex”

Patients travel to us when their case is not straightforward: a full-mouth rehabilitation or multiple implants; bone loss they were told rules out implants; previous implants, crowns or root canals that have failed; or simply the wish to have everything rebuilt without any metal.

These are exactly the cases Dres. Castillo take on — personally, with the surgical training to match. Meet the surgeons who will treat you →

Surgeon-led, never outsourced

Your surgery is carried out by Dr. Alejandro Castillo — Paris-trained oral and maxillofacial surgeon and implant-surgery specialist — with Dr. Carlos Castillo. No associates, no intermediaries.

Bone loss is not a dead end

When there is not enough bone, we rebuild it with autologous bone — your own bone, the gold standard in reconstructive surgery — so a missing foundation doesn’t rule out a fixed result.

Metal-free, with every option

Full-arch zirconia implants and ceramic restorations for a fully metal-free result — and titanium too, when it is genuinely the best choice for you. An informed decision, not a fixed formula.

Rebuilding a mouth from a difficult starting point

Many complex cases begin by undoing previous work. We safely remove failed or corroded titanium implants, metal endodontic posts and amalgam fillings, and treat the underlying infection — often with ozone therapy to disinfect and support healing.

Where the bone has been damaged or lost, we regenerate it with the patient’s own autologous bone, then rebuild function and aesthetics with zirconia implants and ceramic restorations. Every stage is planned in advance and followed closely afterwards. You can read the full clinical detail on our biological restorative surgery page.

A real complex case

From corroded titanium to a metal-free mouth

Wendelyn travelled from Switzerland with a difficult case — infected root canals, corroded titanium implants and metal-ceramic crowns. After a clear plan agreed before she travelled, she had a full metal-free rehabilitation with us in Madrid.

Frequently asked questions

Complex reconstruction: your questions answered

Yes. We carry out complete, full-arch rehabilitations using zirconia (ceramic) implants and ceramic restorations, for a result that is entirely metal-free. Where titanium is genuinely the best option for your case, we place it too — the choice is an informed clinical judgement, not a fixed formula.

The doctors themselves. Dr. Alejandro Castillo is a physician and Paris-trained oral and maxillofacial surgeon and a specialist in implant surgery; his son Dr. Carlos Castillo is trained in implantology, maxillofacial surgery and bone regeneration. Complex surgery is never outsourced to associates.

Often, yes. When there is not enough bone, we rebuild it with autologous bone — the patient’s own bone — which is the gold standard in reconstructive oral surgery. Bone loss alone does not rule out a fixed, metal-free reconstruction; it is assessed case by case from your X-rays.

Yes. We safely remove failed or corroded titanium implants, metal endodontic posts and amalgam fillings, regenerate the affected tissue and rebuild the mouth with metal-free materials. This is one of the most common reasons international patients travel to us for a second opinion.

Yes. “Biological” refers to the use of metal-free, biocompatible materials and a minimally invasive approach — not to a lack of surgical rigour. Bio Salud Dental is surgeon-led, with Paris maxillofacial and implant-surgery training and more than 30 years of experience with complex, full-mouth cases.

It depends on your case, but treatment is planned to be completed in as few visits as possible, concentrated over a short stay. You receive the exact number of days and appointments, along with a fixed quote, before you travel.
